WEEK #139 JPATS w spreadsheet

Welcome to Week #139.
Disasterville General Hospital is going to transfer some patients to Hazard County Memorial Hospital. You will receive a spreadsheet containing the patient information attached to a Winlink email from SNJWW. You will insert that information into a JPATS Form (Select Template/Standard Templates/General Medical Forms/JPATS with viewer)
[Let me know if you don’t get a copy of the spreadsheet at SNJWW, and I’ll send you one ASAP]
Insert the following information in the form’s header:
Incident Name: Disasterville Flood
Op Period: 8/9 -8/10; 0600L-2359L
Site Name: Disasterville General Hospital
Mail your completed form to: SNJMED
Post on groups.io if you need help with the spreadsheet.
